Cosmetic Interventions Need Tighter Controls

The Department of health has commissioned a review of Cosmetic Surgery led by the NHS medical director Bruce Keogh. The results of this review were published yesterday.

This enquiry was precipitated by the PIP breast implant scandal. The senior officers of the French company are now standing trial in the south of France. The scandal is a result of a total failure of government regulation.

The enquiry has now looked at nonsurgical procedures such as dermal fillers and Botox.

It found that there is little or no regulation of these  procedures despite their popularity. The review’s final report said that production of dermal fillers should be subject to the same controls as other implants to ensure that only those that have passed rigourous safety step tests were available and that only practitioners who are properly trained and regulated could administer them.

Currently dermal fillers fall under the general product safety regulations part of the European Union’s directive on product safety, which covers products such as toothbrushes and electric plugs.

In Europe about 200 types of filler are in use, whereas the United States has 14 because the US Food and Drug Administration regulate these products properly.

At last the penny has dropped!

At the Aesthetic surgery clinic we have always restricted our use of dermal fillers to those that have FDA (Food and Drug Administration) approval. The treatments are always carried out by a doctor trained in the technique whereas in many clinics or spas the treatments are carried out by non-medical personnel which in our view is a disgrace.

Platelet Rich Plasma Therapy- The ‘Vampire Lift’

At the Aesthetic Surgery Clinic we have a very conservative attitude towards new treatments. We like to see good evidence that the treatment is safe and effective.

Dracula Therapy aka ‘The Vampire Lift’ would appear to be just such a treatment. The proper name for this treatment is “Platelet Rich Plasma Therapy” or PRP for short. The first publication for this therapy was in connection with  Dental and Oral maxillofacial surgery. It was injected to stimulate bone regrowth.

For the treatment, the patient’s own blood is used. 20ml. of blood is taken from a convenient vein in the arm and then spun down in a centrifuge. The red blood cells sink to the bottom and the supernatant clear fluid is platelet enriched plasma that is used for the treatment. This fluid can be injected into any part of the body to stimulate healing and regeneration. As mentioned before it was first used in dental surgery but is also used to accelerate healing in serious burns, for sports injuries, and even in total knee arthroplasty.

We are very excited by the possibilities for facial rejuvenation. There are many studies that show a very good  improvement in skin condition. It is an extremely safe treatment, and there is no possibility of allergy because it is the patient’s own blood that is being used. Sometimes multiple treatments are needed.

We are confident that this is going to become an extremely popular treatment.

Thoughts on New Royal College of Surgeons Guidelines

Women should not be told that they will look more beautiful after cosmetic surgery and only nurses dentists and doctors should carry out treatments such as dermal fillers, medical leaders say today. Surgeons must be realistic about what they can achieve and must not tell patients that they will look like celebrities, the Royal College of Surgeons says in a new code of practice. Patients must be considered for psychological assessment before surgery and if necessary referred on for counselling, it says. The surgeons also recommend that only specialists should do operations such as breast enlargement.

The college is urging ministers to make the rules compulsory as a review into regulation of cosmetic surgery prepares to report in the spring. The report goes on to say that many patients have unrealistic expectations.

I think that much of this report is a statement of the obvious.

At the Aesthetic Surgery Clinic, Botox treatments are always performed by the doctor who has 20 year experience in the technique.

We are extremely conscientious in managing patients’ expectations.

The Royal College of Surgeons sets standards but cannot enforce them. The guidelines however may become law when Sir Bruce Keogh, medical director of the NHS proposes regulation later this year. We would welcome this but feel that the regulation should go further.

Role of Juvederm and Botox in Plastic Surgery

A branch of Plastic surgery called the cosmetic surgery si mianly used to perform minor operations to improve the features of the face or structures of the body. Many people consider it as an investment to improve their self-esteem. Common treatments in cosmetic surgery include botox, eyelid-reshaping, facelift, brow lift, facial and breast implants, laser hair removal, hair replacement etc. Liposuction and nose job surgeries are of great importance too.

The process of removing facial wrinkles and smoothening them and reducing the folds to make the skin more voluminous so those undergoing this surgery appear younger and more attractive is called Juvederm. It is most commonly performed on nasolabial folds, fine lines and wrinkles surrounding the mouth. The main compound used for juvederm is the Hyaluronic acid is present in our body naturally and its function is to lubricate tissues and joints. The fluid, however diminishes as the person is growing older, making the skin loose and less elastic. The juvederm treatment lasts for about a year. It is mainly used to combat wrinkles and regain the natural tone of the skin. Moreover, a re-treatment can be had at any time, which makes this procedure more popular. The surgery involves injection of the Hyaluronic acid, which can be taken between 15-30 minutes. At the beginning of the surgery, topical numbing cream is applied on to the specific area where the modification is to be made and then juvederm is injected in to those areas gradually and by soothing massages to the treated area. Complete results manifest themselves in 2-3 days accompanied by minor swelling, pain and redness after the treatment.

Juvederm is better compared to other injections as it fills the winkles but it cannot be used for active wrinkles like glabellar lines and forehead corrugations. Hence, Botox is used for these procedures. Juvederms use is limited to the areas around the eyes, lips smile lines, deep wrinkles and scars.

Botox is an injectable that is used commonly in cosmetic surgery. It is a safe treatment method and has very few side effects compared to other fillers. Patients undergoing botox filling recover quickly and this makes the dace look firm and young. Another anti-aging injectable used is a drug called Aquamid. It is a soft volume filler used for heightening cheekbones and improving the contours of the face. It is biocompatible but not approved by the FDA.

The Tumescent Liposcuplture approach to the cross-suctioning of the fatty tissue more delicately. The process employs only smaller incisions and involves less bruising. It is called “liposculpture” because the surgeon is granted with the ability to sculpt the body shape of the patient according to their desire. The regions of neckline, arms, stomach, buttocks, hips, enlarged male breasts can be treated using this therapy. It is to be noted that this method is not a weight loss substitute. Rhinoplasty

Rhinoplasty is a surgery for correcting and reforming nasal structure by restoring the functions as well as aesthetically enhancing the shape of the nose. It is done by resolving the nasal trauma. In many case, all of the incisions will be positioned inside the nose, where they will not be noticeable. If the bottom of the nose is tapering or the nostrils appear reduced, small block of skin at the nostrils will be removed. Incisions are concealed in the natural crease at the point where the nostril links the cheek.

Numbness of the skin after the rhinoplasty may occur but it is generally temporary. The skin of the nose is alienated from the supporting bone and cartilage that is sculpted to the preferred shape using the technique implemented by the surgeon. A little blood loss is common in the early days after surgery and the patient may be insisted not to blow his nose for at least a week to allow the tissues to cure.

After all dressings the stitches are removed. For 2 -3 weeks the patient shall be asked to avoid strenuous movement with no contact point for 6 weeks at least. You will have everyday follow-up visits in the months after surgical procedure to make sure of the progress. This process involves the reshaping of the cartilage bone and occasionally the soft tissues also. This surgery helps to change the angle involving the nose and the upper lip.It helps to get a masculine or feminine look, and correct certain breathing problems also.
